2. Limits of Confidentiality/ Cancellation Policy
   

Please complete the online client intake form and bring the signed confidentiality agreement form with you to the session.

If you would like the psychologist to coordinate your care with another provider (for example, your psychiatrist, primary care general practitioner, etc.), complete this form to authorise release of information:
